Metastatic colon cancer treatment

Apecitabine an oral fluoropyrimidine, which is a three-step enzymatic conversion to 5-FU subjected to the last step is done in the tumor cells. In patients with metastatic colorectal cancer, two studies, the equivalence of capecitabine to 5-FU-leucovorin demonstrated. [24,25] European multicenter study compared capecitabine (1250 mg / m 2) twice daily for 1 to 14 days, then get all 21 days eight cycles of the Plan Mayo Clinic 5-FU and low dose leucovorin in patients with stage III colon cancer. [26] The study showed that the disease-free survival (DFS) at 3 years equivalent to patients, the capecitabine or 5-FU-leucovorin (HR = 0.87, p <.001). [26] [Level of Evidence: 1iiDii] Hand-foot syndrome and hyperbilirubinemia significantly more frequent in patients who capecitabine, but diarrhea, nausea or vomiting, stomatitis, alopecia, and neutropenia were significantly less frequent. Patients receiving capecitabine, 57%, a dose adjustment is required. In patients with stage III colon cancer, which is scheduled to treatment with 5-FU, leucovorin, capecitabine is an equivalent alternative. Oxaliplatin has significant activity in combination with 5-FU-leucovorin in patients with metastatic colorectal carcinoma. In 2246 patients with resected stage II and stage III colon cancer in the MOSAIC study, toxic effects and efficacy of FOLFOX4 in comparison with the same 5-FU-leucovorin regime without oxaliplatin administered for 6 months. Preliminary results of a study with 37 months follow-up showed a significant improvement in DFS at 3 years (77.8% versus 72.9%, P = .01) in favor of FOLFOX4. When reporting, there was no difference in overall survival. Patients treated with FOLFOX4 experienced more frequent toxic effects consisting mainly of neutropenia (41%> grade 3) and reversible peripheral sensory neuropathy (12.4%> grade 3). These results are preliminary and gaps in relation to the OS. These data suggest that FOLFOX4 is resection a therapeutic option for patients with colorectal cancer in stage III.
